GUIDELINES FOR PERSONAL PROFILE

• Please keep the format as per the templet.
• Epasa requires three printed and bound copies for:
  - The Department of Health
  - Getmed Medical Insurance
  - EPASA
• Deliver or post all three copies to EPASA
  (Address below).
• Please type neat, check spelling and vocabulary, and keep the font-type the same (Tahoma).
• Insert photos of yourself and/or other important events covered in your profile.
• No e-mail profiles will be accepted, and members will not be registered with the Department of Health or Getmed if a profile has not been submitted.
• Case studies must be of real patients, with first name (not surname) contact details for possible verification.
• Please phone the EPASA office if you require any additional information or assistance:

  9am - 3pm: Monday to Friday.
  You may also phone Mervyn Bartholomew on 083 266 4338 if urgent.
  
• Should you wish to see any EPASA official for asistance, please make an appointment for a Thursday.
• Should you wish to deliver your profiles to the EPASA office, please phone Princess to ensure that there will be someone to receive your documents.
• Please assist us in submitting the profiles as soon as possible, as we would like to submit them to the various parties before the end of September.
• Ensure that your membership fees are paid up to date for EPASA, and if you are registered under Ethnopsychology, you are required to be registered with both EPASA and HEPASA.
  Profiles will only be submitted to the Department of Health & Getmed for members who have paid them membership fees up to date.
  
• HEPASA (Hypnotherapy & Ethnopsychology Association of South Africa) is aligned under EPASA.
